If you need a MyChart Activation Code, please call your primary care physician's office and request this information. They will be able to assist you and provide you with the information you need. 

Can't Remember your Username or Password? Or locked out of MyChart?

If you have forgotten your MyChart username or password, or you have tried unsuccessfully to login to your MyChart account, please call your primary care physician's office, and they will be able to assist you.

MyChart Mobile

MyChart Mobile is now available to you on your Apple iPhone®, iPod touch® or iPad® or on your Android™ phone! This does not replace the web-based MyChart and does not include all of the same features – and you will need to have a MyChart account before you can access MyChartMobile.

Once your MyChart account is set up, here’s how to access MyChartMobile:

  1. Download the free MyChart app from the Apple App Store or from Android Market.
  2. Once you’ve launched the app, select the following entry from the list of healthcare providers. If you have a MyChart account, your physician’s office is part of Mercy Health.
  3. Enter your MyChart username and password and begin using MyChartMobile!

Please contact the physician’s office where you signed up for MyChart if you have difficulty with accessing your medical information or using MyChart.

Please note – MyChart should not be used for urgent medical Matters!

Please do not use MyChart to send any messages requiring urgent attention. For medical emergencies please call 911. For urgent medical matters, call your doctor's office.
